REGULATIONS FOR THE PURCHASE AND USE OF GIFT VOUCHERS AT DR. FRANKEL’S

The following are the general terms and conditions for the purchase and use of gift vouchers. Dr. Frankel’s Clinic reserves the right to interpret, modify or rescind these terms and conditions without notice. 1. A Gift Voucher is a document issued by Agnes Frankel, with its registered office in Warsaw, at 15/4 Raszyńska Street, enabling the realization of selected services provided by Dr. Frankel’s NZOZ in Warsaw, at 15/4 Raszyńska Street (hereinafter referred to as the Clinic), allowing payment for services provided by the Clinic in accordance with the applicable Regulations, price list and the value of the Gift Voucher held.

2. The Gift Voucher is issued in the form of a paper invitation.

3. The Gift Voucher shall contain: the name of the recipient (Beneficiary), the name of the service or the corresponding amount to be used, the expiration date, the registration number. The Gift Voucher may also contain a description of the method of its implementation.

4. Any changes to the content of the Gift Voucher, including deletions, cross-outs, additions, additions shall render the Gift Voucher invalid.

5. The Gift Voucher may be purchased at the Clinic’s reception desk, or in the online store at http://shop.drfrankels.com.

6. the Gift Voucher is valid for 12 (twelve) months – counted from the date of its issuance – during this period you must use the services based on the purchased Gift Voucher. After the expiration date – the Gift Voucher becomes invalid, and the holder of the Gift Voucher loses the rights arising from it. If the holder fails to redeem the Gift Voucher or wishes to use the Gift Voucher after the expiration date of the Gift Voucher, the holder of the Gift Voucher shall not be entitled to any rights under the Gift Voucher, nor shall the holder claim any right to compensation or refund of the Gift Voucher fee.

7. The expiration date of the Gift Voucher shall be extended only if the performance of the services covered by the Gift Voucher is not feasible within a period of 12 months due to the time intervals between treatments provided in the treatment procedures. In such a situation, the expiration date of the Gift Voucher is extended by the time necessary to perform the treatments.

8. The Gift Voucher is not refundable or exchangeable for cash.

9. When the value of the Gift Voucher is higher than the value of the service performed with its use, the resulting excess is not refundable.

10. As requested, the recipient may pay extra to the Gift Voucher when the value of the services he wishes to use exceeds the value indicated on the Gift Voucher.

11. The Gift Voucher may be used during one or more visits depending on the type of services.

12. When making an appointment for a consultation and/or treatment, the Beneficiary should inform that he/she is using the Gift Voucher.

13. Services provided under a Gift Voucher may not be combined with other discounts, rebates or promotions.

14. the Clinic has the right to refuse to perform a selected treatment/package resulting from a purchased Gift Voucher, in case the Clinic’s staff determines that there are contraindications to performing such treatment.

15. if the Gift Voucher was issued for specific services, the Clinic allows the possibility of exchanging the service so purchased for another/other services of the value indicated on the Gift Voucher, if there are significant contraindications to performing the service indicated on the Gift Voucher.

16. In order to use the Gift Voucher, it is necessary to contact the Clinic’s reception, make an appointment for a specific date in order to perform the service, together with indicating the number and expiration date of the Gift Voucher to be used.

17. In order to use the Gift Voucher, it is necessary to give it in original to the Clinic’s receptionist before performing the planned service.

18. The Clinic reserves the right to refuse to redeem a Gift Voucher used contrary to the provisions of these Regulations.

19. In order to cancel an appointment, it is necessary to contact the Clinic reception by phone at +48789375101 at least 48 hours before the appointment. In this case, the Beneficiary will not bear the cost of cancellation.

20. Failure to cancel an appointment by the Visitor at least 48 hours before the scheduled treatment will be considered a completed appointment without the possibility of claiming a refund for the value of the planned treatment.

21. Dr. Frankel’s Clinic reserves the right, in exceptional situations, to cancel all reservations for reasons beyond its control. In such case, a new appointment will be scheduled, and the Beneficiary will not bear any costs.

22. The Beneficiary has the right to file a complaint regarding the services provided by the Clinic. The complaint should be submitted in writing to the e-mail address: recepcja@drfrankels.pl. The Clinic undertakes to consider the complaint within 14 working days of its receipt and inform the Beneficiary of the result of the consideration. When filing a complaint, the Beneficiary is required to provide his/her personal data, address data and telephone number, which will be processed in accordance with RODO, for the purpose of the complaint process. The provision of personal data is completely voluntary, but necessary for the effective implementation of the complaint process.

23. These Regulations are an integral part of the Gift Vouchers. When making a purchase and before redeeming the Gift Voucher, the person purchasing the Gift Voucher and the person redeeming the Gift Voucher declare that they have become familiar with its contents and agree to abide by them.

24. The person purchasing the Gift Voucher also undertakes to inform each user of the Gift Voucher of the contents of these Terms and Conditions and the places where the contents of the Terms and Conditions may be consulted. 25 The user of the Gift Voucher declares that he/she agrees (in accordance with Art. 6 paragraph. 1 point a of Regulation (EU) 2016/679 of the European Parliament and of the Council of April 27, 2016. on the protection of individuals with regard to the processing of personal data and on the free movement of such data and the repeal of Directive 95/46/EC (General Data Protection Regulation) (hereinafter: “RODO”)) for the Clinic to process the personal data provided in order to properly exercise the rights and obligations arising from the Regulations, including for the purpose of conducting the complaint process. The Clinic declares that the personal data will be collected and processed by the Clinic as the Administrator of the personal data in accordance with the RODO and will be used only within the framework of and for the purposes of the proper realization of the rights and obligations under the Regulations, including for the purposes of carrying out the complaint process, and for the protection or vindication of possible claims, hereinafter referred to as “the Administrator’s legitimate interest”.
